After completing the enrollment form, Qualified Health will send you an assortment of information concerning the who, what, where, and how of affordable healthcare. With your new Member ID Cards and the information you receive from Qualified Health, you are on the fast track to claiming your right to affordable quality healthcare.

You will receive your Membership Handbook which includes all necessary information and phone numbers. Saving is simple: Call Customer Service to locate a participating provider, alternatively, click here to locate a provider. Then, call the chosen healthcare provider to schedule an appointment. At the appointment, show your Qualified Health ID card and identify yourself as a member of the applicable network. Savings should be applied immediately to your current visit. Qualified Health's healthcare program can be used to together with existing insurance programs and major medical policies. Qualified Health also works with policies that pay a flat fee for hospital stays or certain conditions. It is important to realize that our program cannot be used with PPO's, HMO's and other managed healthcare plans with small co-payments at the time of service.
